Submitting and Following Up

Protecting sensitive information

Each resource should identify whether it can be downloaded, completed online, uploaded through a secure portal, or submitted through another approved method. Follow the submission instructions attached to the applicable referral form.

Do not send protected clinical records, Social Security numbers, financial information, or other unnecessary sensitive information to a general StrongSteps email address unless StrongSteps has specifically established an approved secure process. If you’re unsure how to submit documentation, call (888) 604-5527.

Already made a referral?

You do not need to submit another referral just to ask about the first one. Have your referral confirmation number, the individual’s name, the date submitted, your referring organization and your contact information available.

Use the designated referral-status process or call (888) 604-5527. Information StrongSteps can provide may depend on applicable privacy and authorization requirements.
