ELIGIBILITY

Finding the Right Support Starts With Understanding Your Options.

StrongSteps programs are designed for different needs and circumstances. You do not need to understand every requirement before reaching out.

Eligibility Information Is a Starting Point

The information on this page provides general guidance. Final eligibility may require a conversation with StrongSteps, review of documentation, confirmation of coverage or funding, an assessment, a referral, and review of current program capacity.

Meeting general eligibility requirements does not automatically guarantee enrollment or immediate access to services.

You do not need to determine your own eligibility before contacting StrongSteps.

Eligibility Is Different for Each Program

Age

Some programs may serve adults, young adults, or other defined age groups. Each program page lists its exact age range.

Location

Availability may depend on the county, ZIP code, region, office location, or approved service area.

Individual Need

A program has to be a good fit for the person's current needs, goals, and circumstances.

Referral Requirements

Some programs allow direct contact or self-referral. Others accept referrals from a family member, provider, hospital, health plan, or Social Security.

Coverage or Funding

Eligibility may depend on Medicaid, managed care, insurance, grants, government benefits, contracts, or program-specific funding.

Documentation

We may ask for identification, proof of residence, coverage or benefit information, referral forms, consent forms, or a release of information.

Program Capacity

A person may meet the general requirements while a program has limited openings or a waitlist. We'll always explain the next step.

How to begin

Community Resource Navigation

When someone's dealing with more than one system at once (behavioral health, housing, benefits) and it's hard to know where to start, this program is built for exactly that. We help sort through the options and point toward what actually applies to them.

After a referral comes in, our navigators reach out to understand the person's specific situation before pointing them anywhere. It's not a generic resource list, it's tailored to what they're actually facing.

A referral form is the fastest way to start. Let us know what the person is navigating, whether that's housing, benefits, healthcare, or several at once, and we'll take it from there.

Peer Support & Community Stabilization

When someone needs steady, day to day support from someone who's been through similar experiences, this program is built around that connection. It's not crisis response or clinical treatment, it's practical help with routines, stability, and staying connected to resources.

Our peer support team reaches out after a referral comes in to talk with the person directly and see if this kind of ongoing, relationship based support is the right fit for them.

Submit a referral and note that you think peer support or community stabilization fits. If you're not sure it's the right level of support, get in touch first and we can talk it through.

Health Insurance Navigation

Anyone who needs health coverage or isn’t sure what they qualify for. That includes people who’ve lost coverage, had a change in income, job or family, or are trying to make sense of Medicaid, Child Health Plus or the Essential Plan. It’s free, and you don’t need to be a StrongSteps client.

No referral needed. People can reach out on their own, and providers or family members are welcome to pass along our number. A Navigator sits down with the person one on one, walks through their options in plain language and helps them enroll through NY State of Health.

Call (888) 604-5527 to set up an appointment. Bring ID, proof of income and any current insurance information if you have it, but if something’s missing, come anyway and we’ll start with what you have.

What Does "Eligible" Mean?

01

Initial inquiry

You reach out, or a provider submits a referral. Eligibility means someone may meet the general requirements for a program.

02

Eligibility review

We look at age, location, need, referral requirements, and program criteria.

03

Referral, documentation, and assessment

Some programs need referral forms, records, consent, or a service-need review before a decision.

04

Coverage or funding review

We confirm Medicaid, insurance, grant, or other funding requirements when they apply.

05

Program decision and next step

You'll get either enrollment steps or a clear next-step recommendation. Requirements and review processes vary by program.

You Don't Have to Figure It Out Alone.

You may know what kind of support you need without knowing the name of the program. Tell StrongSteps what you are looking for, and we can help you explore possible options.

StrongSteps programs may serve different counties and geographic areas. A program available in one county may not be available in another.

Frequently Asked Questions

Do I need to know if I'm eligible before I call?

No. Tell us what's going on and we'll help you figure out which programs might fit.

Each program has its own rules about age, where you live, what kind of support is needed, and how it's funded. Some also need a referral or certain coverage.

Usually not. Community Resource Navigation doesn't require insurance, and if you don't have coverage, our Health Insurance Navigation program can help you get it for free. We'll tell you what applies to any program when you call.

We may still be able to share information and point you to resources closer to you. View Service Areas

We'll do our best to point you toward another program or community resource that might help.

Not always. Some programs have limited space. If a program is full, we'll explain your options and what to expect.
