Serving Communities Across New York State

StrongSteps Behavioral Health Foundation serves communities across New York State.

Our statewide approach allows StrongSteps to build relationships with individuals and families, behavioral-health providers, healthcare organizations, hospitals and health systems, Social Care Networks, peer and recovery organizations, housing organizations, schools and colleges, community-based organizations, nonprofits, businesses, government partners, and other community organizations.

Our goal is to help strengthen connections between behavioral health, social care, recovery, and community support throughout New York.

StrongSteps serves communities throughout New York State. However, individual programs may operate within specific counties, regions, payer networks, funding areas, approved service areas, contract areas, or program jurisdictions.

Program availability may also depend on staffing, capacity, funding, regulatory approval, insurance participation, Medicaid requirements, managed-care participation, program authorization, and referral requirements.

Always verify the specific program before making a location-dependent referral. Determine availability based on the specific program and the individual’s location, rather than assuming that the presence of a StrongSteps office means every service is available in that area.

CORE Service Areas in Detail

Community Support

StrongSteps Community Support programs may operate across different geographic areas depending on the specific service, funding source, and program requirements.

Care Coordination availability depends on the specific StrongSteps program and applicable eligibility, funding, payer, and geographic requirements.

Community Navigation may be available through different StrongSteps programs, partnerships, or funding pathways. Availability and eligibility may vary by community.

Availability may depend on location, referral source, program, transition setting, eligibility, funding, and current capacity. Providers planning hospital or program discharges are encouraged to contact StrongSteps early when possible.

Youth and Young Adult programs may have specific age ranges, geographic areas, funding requirements, consent requirements, and program criteria. Not every StrongSteps behavioral-health program serves minors.

Peer and Recovery Support may be available through different StrongSteps programs. Some peer services may be Medicaid-funded and have specific service areas; other community peer initiatives may have different geographic requirements.

Housing and basic-needs resources vary significantly between communities. StrongSteps may help individuals navigate available resources, but availability of housing, rental assistance, shelter, food programs, transportation, utility assistance, financial assistance and other community resources depends on the individual program, location, eligibility, funding, and community resource availability. StrongSteps cannot guarantee that a particular resource will be available in every county.

Two Things to Check Before You Refer

Service area vs. eligibility

Service area asks: is the StrongSteps program available where the individual lives or receives services? Eligibility asks: does the individual appear to meet the program’s requirements?

Someone may live within a service area but not meet eligibility requirements. Likewise, someone may appear eligible for a type of service StrongSteps does not currently provide in their area. Check both before referral whenever possible.

Service area vs. capacity

Even when a program operates within a particular area, immediate enrollment may depend on current capacity — staffing, caseload capacity, funding, authorization, payer participation, program resources and referral volume.

A county listed within a service area should not automatically be interpreted as a guarantee of immediate enrollment.
