You can begin by completing a New Client Registration form, contacting StrongSteps directly, or asking an eligible provider to submit a referral.
We'll ask for enough information to understand the request and identify possible programs. You do not need to know the name of a StrongSteps program before reaching out.
A member of our team reviews what you shared to determine what program, staff member, or next step may be most appropriate.
We may contact you, a caregiver, or the referral source by phone, secure email, or secure message to ask questions and discuss next steps.
Each program may have different requirements. We'll explain which criteria apply and whether more documents, assessments, or referrals are needed.
The exact process may vary depending on the program, referral requirements, location, funding, and individual needs. We'll tell you where things stand.
Full legal name, preferred name, date of birth, phone, email, address or ZIP code, preferred language, and preferred contact method.
Current concerns or needs, current providers, and any recent transition or hospital discharge.
Anything about housing, food, transportation, or accommodations that would help us understand the request.
Insurance or Medicaid information if applicable, referral source, program documentation, and a release of information when required.
Guardianship or custody information when applicable, contact information for the child or young adult, school or provider information, and consent documentation.
You'll see on-screen confirmation that your registration was submitted successfully.
A StrongSteps team member reviews the information and determines the appropriate follow-up. Submitting a form does not guarantee eligibility or enrollment.
We contact the person named in the registration using their preferred method whenever possible. Response times may vary based on staffing, program requirements, urgency, and the information provided.
We collect only the information needed to review your request, talk with you about possible services, and complete required intake or referral steps.
Only provide sensitive or protected information through an approved secure form. Do not send medical records through a general email address unless we have confirmed the method is secure.

Family members, caregivers, providers, and community members may contact us for general information or help beginning a request.
Privacy, age, guardianship, and consent requirements may affect what we can discuss or complete on another person's behalf. For a minor, a parent or guardian may need to give consent. For a client or patient, a provider may need a release of information.
StrongSteps registration and referral forms are not monitored as emergency services.
If there is an immediate danger, medical emergency, or threat to someone's safety, call 911 or go to the nearest emergency department.
For mental health, substance-use, or suicide-related crisis support, call or text 988.

Begin a registration, explore support options, or contact StrongSteps if you are unsure which path is right for you. Reaching out is the first step. StrongSteps will help explain what comes after that.
